Health/Medical Insurance

To file your Health/Medical insurance claim, please follow the steps below:

• Download, fill out the Claim form for reimbursement of expenses and sign
• Send the scanned copy of the Claim form for reimbursement of expenses, together with copies of all initially required documents by the Insurer and send via email at: claims@axiom-jsc.com or by courier addressed to: 1408 Sofia, 150 Vitosha Blvd., Axiom Insurance Company.

Claims with an invoice value of up to BGN 500.00 under health/medical insurance are processed without the need to submit original financial documents. Please download, fill out and submit scanned copy of the Declaration to the Claim form for reimbursement of expenses at the amount of up to BGN 500.00 (five hundred), therefore the Insurer reserves the right to request the original financial documents for review during the claim assessment process.
